Our Process

On the day of your appointment, have your referral (if given), insurance information, and a summary of your medical concern and history ready to speed up the intake process.

Internists act as a primary point of contact for adult medical needs, and Dr. Amir Alizadeh conducts thorough evaluations for both sudden and ongoing health concerns. He helps make sense of complex symptoms, manage conditions, and connect you with specialists when needed.

Contact us or your health plan's member services to confirm if we are in-network and authorized to serve as PCP (Primary Care Provider) for your health plan. 

Obtain and complete any required PCP designation forms, or update your selection through your health plan’s member portal, providing Dr. Amir Alizadeh’s full name, practice address, and National Provider Identifier (1942333877) if requested.

After submission, confirm the effective date of the change from the health plan that your member record is updated, designating Dr. Amir Alizadeh as your PCP.

Most foundational screenings are fully covered by insurance plans, with several free state programs available for uninsured and underinsured residents in Rhode Island.


  • Blood Pressure - For adults 18+ at least once every 2 years or annually if elevated.
  • Cholesterol (Lipid Panel) - For adults 20+ every 4 to 6 years for average-risk individuals.
  • Diabetes - For adults 35-70 every 3 years or earlier if overweight or has a family history.
  • Bone Density - For adults starting at 60, every 2 years if low risk or annually if you have active osteoporosis.
  • Full Physical Exams - For adults over 40, including mental health screening annually.
  • Cancer Screenings - For adults starting at 40, for skin, lungs, breast, digestive, and reproductive organs.

The Rhode Island Department of Health recommends staying up to date with routine vaccinations to protect your health year-round, as well as one-time special vaccinations based on age or underlying conditions.


Routine Vaccinations


  • Annual influenza (flu) vaccine - Recommended every fall to protect against the season’s circulating strains.
  • COVID-19 vaccine - Receive the latest formulation/booster as recommended to maintain protection against current variants.
  • Tdap vaccine - One-dose Tdap as an adult, then a tetanus/diphtheria (Td) or Tdap booster every 10 years as advised.

Age and Risk Based Vaccinations


  • Shingles (herpes zoster) vaccine - Recommended for adults aged 50 and older as a one-time series to prevent shingles and complications.
  • Pneumococcal vaccine - Consider starting as early as age 29 if you have risk conditions and discuss timing and frequency with your physician.
  • RSV (respiratory syncytial virus) vaccines - Recommended for adults aged 60 and up when eligible formulations are available and advised.
  • Hepatitis vaccines - Recommended for adults with risk factors or those aged 59. Discuss options with your physician.
